Findings: On or about January 3, 2021, the Alleged Victim (AV) heard a knock on a door, went to answer the door and fell. AV was taken to the hospital and diagnosed with a broken hip. AV only had one shoe on at the time of the fall, but is care planned to have non-skid shoes on while ambulating. The facility failed to follow AV's care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01641 $375.00 fine assessed
