Findings: The facility neglected Alleged Victim (AV) by failing to provide adequate nutrition to AV, which resulted in harm. On or about December 27, 2020, AV was sent to the hospital for failure to thrive and poor oral intake. The facility's failure to provide oversight and monitoring of change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
