Findings: On or about January 5, 2021, Alleged Victim (AV) and Witness 1 (W1) were involved in a resident-to-resident altercation. An investigation determined that W1 had been involved in two (2) other resident-to-resident altercations within the month prior to the day of the incident. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02355 $375.00 fine assessed
