Findings: Alleged Victim (AV) has a known history of falls. From October 14 through November 26, 2020, AV had approximately six (6) falls with injury. On or about January 2, 2021, AV had a witnessed fall while ambulating in the dining room, AV was using a walker at the time of the fall. The facility failed to properly plan care and mitigate the risk of falls to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
