Findings: Alleged Victim (AV) was care planned to have his/her wheelchair next to his/her bed. On or about January 7, 2021, AV fell from his/her bed and was found halfway on the floor mat. AV’s wheelchair was not placed beside his/her bed. The facility failed to follow AV’s care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
