Findings: The facility failed to provide the basic care and services necessary for the safety of Alleged Victim (AV) which resulted in AV experiencing nine (9) falls in eleven (11) days causing two (2) head injuries, abrasions to both knees and one arm, a fracture to his/her right humerus, and a fracture to his/her fifth metacarpal. An investigation determined that the facility failed to implement reasonable interventions to address AV's increasing and ongoing falls which included falls with injury. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02314 $2500.00 fine assessed
