Findings: On or about January 12, 2021, it was discovered that Alleged Perpetrator #2 (AP2) was removing narcotics from the med room, performing audits and destroying medications on his/her own without another staff member present to witness the destruction of medications. The Alleged Victim (AV) still had a valid as needed prescription for the medication destroyed and missed medications due to them being destroyed by AP2. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
