Findings: On or about January 12, 2021, it was discovered that the Alleged Victim (AV) had missed his/her narcotic medication, but there is no explanation as to why he/she missed his/her medication. This action placed AV at risk for harm. An investigation determined the no Alleged Perpetrator #2 wrongdoing or abuse occurred in this case. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
