Findings: On or about January 12, 2021, it was discovered that Alleged Perpetrator #2 (AP2) was removing narcotics from the med room, performing audits and destroying medications on his/her own without another staff member present to witness the destruction of medications. The Alleged Victim (AV) had an active narcotic medication which was active until November 23, 2020, however AP2 marked destroying the medication on November 13, 2020, 10 days prior to the medication being changed to a liquid narcotic. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
