Findings: On or about January 12, 2021, it was discovered that Alleged Perpetrator #2 (AP2) was removing narcotics from the med room, performing audits and destroying medications on his/her own without another staff member present to witness the destruction of medications. The Alleged Victim (AV) had narcotic medication left that was marked as destroyed, there is no second signature on the documentation to show that the medication was actually destroyed. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
