Findings: On or about January 12, 2021, it was discovered that Alleged Perpetrator #2 (AP2) was removing narcotics from the med room, performing audits and destroying medications on his/her own without another staff member present to witness the destruction of medications. The Alleged Victim (AV) had a medication that was checked in by AP2 at a level less than what is supplied by the provider, and documented a destruction of medication at 5 ml, indicating that it couldn't be retrieved from the bottle. There was no secondary staff to verify this was the case. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
