Findings: On or about August 8, 2020, Alleged Victim (AV) wrote a check to the facility for his/her rent. In November 2020, AV received notification from his/her bank of non-sufficient funds. AV requested a copy of transactions. Alleged Perpetrator 2 (Unknown staff member) had taken a copy of AV's check, altered the check and attempted to complete transactions. From October 19, 2020 through October 23, 2020, a total of 6 checks were written for a total dollar amount of $14,500.00. Checks were also attempted to be deposited. AP2's actions are considered financial exploitation and constitutes abuse. The facility failed to protect AV from financial exploitation which is a violation of Oregon Administrative Rules.
