Findings: Alleged Victim (AV) is minimally oriented and relies on facility staff to ensure that his/her personal care, safety, and supervision requirements are met. Witness 1 (W1) has minimal memory and experiences declining orientation. On or about January 16, 2021, AV was found in W1’s room in apparent sexual behaviors. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
