Findings: The Alleged Victim (AV) was care planned to have a shower and have his/her nails trimmed each Sunday. In January 2021, AV's family arrived at the facility to take AV for a nail appointment. It was discovered at that time that AV's toenails had not been trimmed for a very long time, which caused AV embarrassment and unnecessary discomfort. The facility was to notify family if AV refused showers. It was noted that AV refused many showers, however, family was never notified of the refusals. The facility failed to provide service and follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
