Findings: Alleged Perpetrator 2 (AP2) physically abused Alleged Victim (AV) when AP2 used physical force against AV by grabbing AV's wrist which was healing from a fracture and when AP2 crossed AV's arms across AV's chest which caused AV pain. AV's actions is considered physical abuse. AP2 verbally abused AV when AP2 yelled and cursed at AV when AP2 was assisting AV with his/her ADL's. AP2's action is considered verbally abuse. AP2 restrained AV by leaning against AV so AV could not stand from the toilet and by holding AV's arms and crossing them across his/her chest for the convenience of AP2. AP2's actions is considered wrongful restraint which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
