Findings: Witness 1 (W1) has a known history of inappropriate sexual contact with other residents. On or about January 25, 2021, AV was watching tv in the common activity room when Witness 1 (W1) approached AV, while W1 had a one-to-one staff present and gave AV a kiss on the mouth. The facility failed to protect AV from W1’s known behavior, which is a violation of resident rights, is neglect of care and constitutes abuse.
