Findings: On or about January 25, 2021, the Alleged Victim (AV) left the facility unattended and unnoticed by staff for a couple of hours. At 8:00 pm, staff noticed that AV was not in his/her room and could not locate him/her in the facility. Staff eventually located AV outside by the facility sign, near the road, on the ground from a fall. AV was sent to the hospital and sustained an injury to his/her knee. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
