Findings: Alleged Victim (AV) relies on the facility for care. Between November 20, 2020 through December 11, 2020, AV experienced three falls. On or about January 24, 2021, AV had called for assistance, but no staff responded, which resulted in AV falling and causing bruising to his/her face. AV’s care plan does not reflect current mobility needs. The facility failed to care plan according to AV’s current needs with mobility, which is a violation of resident rights, is neglect of car and constitutes abuse.
Sanction: ALFCP21-01662 $500.00 fine assessed
