- Findings
- The facility failed to provide Alleged Victim (AV) with basic care of assistance with mobility and supervision for safety which resulted in AV attempting to self-transfer in his/her room resulting in AV falling and sustaining a skin tear on his/her elbow. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
