Findings: The facility failed to follow the Alleged Victim's behavior plan to keep away from Witness 1 at all times and staff are to be present if residents are in the same area. The failure resulted in a verbal altercation and W1 pushing AV into the wall, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
