Findings: Witness 1 (W1) was care planned to have a one on one staff. On or about February 4, 2021, W1 and Alleged Victim (AV) got into a resident to resident altercation, causing W1 to sustain scratches to his/her face and a raised area on his/her shin. The incident was not observed by staff since W1 did not have a one to one staff with him/her. The facility failed to follow W1’s care plan of having a one on one staff exposing AV to potential harm, which is a violation of resident right, is neglect of care and constitutes abuse.
Sanction: ALFCP21-02668 $375.00 fine assessed
