Findings: The Alleged Victim (AV) was making trips to the Emergency Department unaccompanied by staff. Staff knew that AV had some overall diminished capacity, however, allowed AV to go alone to the Emergency Department, and failed to update AV's service plan to reflect changes in his/her cognition and ability to make decisions. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00229 $1000.00 fine assessed
