Findings: The facility failed to follow AV's care plan to check on him/her frequently at night ensure proper lighting in AV's room for fall prevention. The failure resulted in AV experiencing an unwitnessed fall and was on the ground for an undetermined period of time as staff stated AV was not checked on that night, AV was diagnosed with a pelvic fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
