Findings: The Alleged Victim (AV) is bed-bound and relies on the facility for management of bowel/ bladder care. AV was found to have redness and a blister forming on his/her thigh, due to staff not changing AV as often as they should. AV would at times resist cares due to staff not taking the time necessary to ensure AV's comfort while changing him/her. AV's care plan did not detail how to change AV, which caused unnecessary discomfort to AV. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
