Findings: The facility failed to provide basic care and services necessary to maintain the health and safety of Alleged Victim (AV) which resulted in AV being abused by another resident of the facility. On or about January 21, 2021, Witness 1 (W1) exposed self to AV while approaching AV in the foyer of the facility which caused emotional harm to AV. The facility failed to provide a safe environment for AV by failing to address W1 being sexually inappropriate in common areas of the facility. The facility's failure is violation of resident rights, is considered neglect of care and constitutes abuse.
