Findings: On or about February 4, 2021, Alleged Perpetrator #2 (AP2) verbally abused the Alleged Victim (AV) by calling AV derogatory names, insulting him/her, using profanity and verbally assaulting him/her. AP2 also failed to follow AV's care plan to change him/her after he/she had an incontinence incident, causing unreasonable discomfort and a loss of personal dignity. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes verbal abuse and neglect. The facility failed to protect AV from verbal abuse, which is a violation of Oregon Administrative Rules.
