Findings: The facility failed to provide Alleged Victim (AV) the basic care and services necessary to maintain AV's health and safety by failing to follow AV's doctor's orders for continuous oxygen. An investigation determined that AV did not receive continuous oxygen as ordered from February 3, 2021 to the morning of February 11, 2021 creating a risk of serious harm to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
