- The Alleged Victim (AV) and Witness #2 (W2) were care planned to be monitored for behaviors. AV and W2 had a verbal altercation and W2 poured juice on AV's head. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
