Findings: Witness 1 (W1) had a known history of aggression and wandering into other resident’s rooms. On or about February 15, 2021, Alleged Victim (AV) was heard yelling, W1 was found in AV’s room holding both of AV’s arms. AV had a half dollar size skin tears and large bruises forming on AV’s hands and a small cut on AV’s pinky. AV stated he/she was very scared by what W1 did and was afraid W1 could come back. The facility failed to properly plan care around W1’s known behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02871 $500.00 fine assessed
