Findings: Alleged Victim (AV) had a known history of aggressive behavior. On or about February 17, 2021, AV and Witness 1 (W1) were in the common area unsupervised and got into an altercation. Staff members reported that the common area should have been supervised by a staff member, but the facility was short staffed. The facility failed to provide supervision and care plan for AV’s known history of behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
