Findings: On or about February 13, 2021, Alleged Victim (AV) sustained an injury when he/she fell out of his/her bed. An investigation determined that the facility failed to implement interventions and appropriately care plan around AV's fall risk which caused AV unreasonable discomfort and caused the development of an open State 2 pressure sore due to the February 13, 2021 fall. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
