Findings: On or about January 21, 2021, Alleged Victim (AV) exposed his/her genitals in common area of the facility while approaching Witness 1 (W1) in the foyer which caused emotional harm to W1. AV has a history of being sexually inappropriate in common areas of the facility. The facility failed to provide a safe environment by failing to address AV's inappropriate behavior which is a violation of resident rights, is considered neglect of care and constitutes abuse.
