Findings: On or about February 25, 2021, during an audit of the medication cart, staff found that 2 narcotic medications had been removed and replaced with tylenol in the Alleged Victim's (AV) medication card. AV's medication had been discontinued on February 9, 2021, however, staff had not removed the medication and destroyed it as should have happened in this case. The medication was replaced by an unknown person, Alleged Perpetrator #2 (AP2), this person is responsible for theft of medication, which constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01557 $375.00 fine assessed
