Findings: On or about January 20, 2021, Alleged Victim (AV) had an unwitnessed fall in his/her room which resulted in AV fracturing his/her right hip. An investigation determined that the facility failed to follow AV's care plan which required one person assist and fifteen (15) minute checks. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02356 $1500.00 fine assessed
