Findings: On or about February 26, 2021, the facility discovered the Alleged Victim (AV) had two (2) full controlled medication cards missing. There had been no administration from those two prescriptions, explanation of destruction or evidence of discontinuance. AP2’s Narcotic count was off and totals were altered to reflect medication that was used up or destroyed which were not documented. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03238 $250.00 fine assessed
