Findings: AV is care planned for nighttime preparation including but not limited to getting into bed, transfer assistance and safety checks twice per night. On or about March 02, 2021, AV was found sitting in his/her recliner with a meal tray in front of him/her. The facility failed to follow AV's care plan causing AV unreasonable discomfort, which is a violation of resident’s rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i)
Sanction: ALFCP21-03012 $500.00 fine assessed
