Findings: Alleged Victim (AV) was readmitted to the facility on or about February 3, 2021 with a left heel wound. The facility was aware AV had a wound on his/her left heel. On or about February 9, 2021, AV was sent to the hospital where it was discovered AV's heel wound needed surgical treatment. The facility failed to address AV's heel wounds and did not provide any treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
