Findings: The Alleged Victim (AV) has a known fall history at the facility and had approximately 13 falls from October to December. On or about December 6, 2020, the AV suffered a fall which resulted in a fractured hip and a mild head injury. The facility failed to properly care plan for falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02478 $1500.00 fine assessed
