Findings: On or about March 4, 2021, the Alleged Victim (AV) and Witness #1 (W1) were involved in a resident to resident altercation where W1 hit AV with a teddy bear during a verbal altercation. Neither resident was injured, however, the residents were placed at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
