Findings: Alleged Perpetrator 2 (AP2) signed out the Alleged Victim's (AV) narcotic medications on several occasions. When AV was asked if the medication was successful, AV would report to staff that she/he did not take any as needed medications. Additionally, documentation was consistently incomplete for administration of as needed medications. AP2's actions are a violation of residents rights and considered financial abuse. Staff reported concerns regarding AP2's possible medication diversion and failed to respond and/or investigate the allegations. The facility failed to provide a safe medication administration system which is a violation of resident rights, is considered neglect of care and constitutes abuse.
