Findings: Alleged Perpetrator 2 (AP2) signed out several of the Alleged Victim's (AV) narcotic medications, without completing all documentation and it is believed that AV did not receive the medications. Facility staff reported concerns around AP2's behavior however there is no documented evidence the facility responded to the concerns. AP2's actions are considered neglect of care and constitute financial exploitation. The facility failed to provide a safe medication administration system which is a violation of resident rights, is considered neglect of care and constitutes abuse.
