Findings: On or about March 6, 2021, it was reported to Alleged Perpetrator #2 (AP2) that the Alleged Victim's (AV) call button was not functional. It is facility policy that when a call button is not functioning that there should be an interim service plan (ISP) in place to check residents 32 times per shift. AP2 did not write an ISP when he/she was advised of the call button not working, leaving AV at risk for harm. On March 8, 2021, AV fell from bed and was found on the floor at 6:30 am by staff. It is unknown how long AV was laying on the floor prior to staff finding him/her. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
