Findings: The Alleged Victim (AV) has a history of falls and is care planned to have assistance with transfers and to be assisted with standing up and sitting / laying down. On or about February 23, 2021, AV was being assisted in his/her room by Alleged Perpetrator #5 (AP5) when AV fell and suffered a fractured hip requiring surgery. AP5 was distracted while AV fell, not following AV's care plan. AP5 had been counseled prior about distracted care giving of residents. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failures are a violation of Oregon Administrative Rules.
