Findings: Alleged Victim (AV) is care planned for staff to ensure that AV has cigarettes upon request. On or about March 17, 2021, at 3:50 am, AV asked for cigarette and was asked to wait. AV became aggressive and broke the fence in the courtyard and eloped through the broken fence. On March 17, 2021, at 5:35 am, AV again requested a cigarette and was asked to wait. AV again became aggressive and begun to throw furniture at staff and placing others at risk of harm. The facility failed to follow AV’s care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00600 $375.00 fine assessed
