Findings: Alleged Victim (AV) is care planned to have staff member accompany AV to his/her appointments. On or about March 17, 2021, AV attended a physician appointment, but no staff member was with him/her. When the transportation service came to pick AV up, he/she was nowhere to be found. AV was found in the back of the building hyperventilating and was very confused. AV was sent to the ER where they administered hydroxyzine. The facility failed to follow AV’s care plan resulting in significant emotional harm and unreasonable discomfort and created the risk of serious harm to AV, which is a violation of rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-01707 $1500.00 fine assessed
