Findings: The Alleged Victim (AV) had multiple falls at the facility, both with and without injury. The facility failed to complete internal investigations on multiple incidences to rule out abuse or report these incidences as required. AV is very impulsive and the facility failed to have enough staff available to ensure AV's safety, leading to falls and a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
