Findings: Witness #1’s (W1) has history of sexual behaviors with residents of the opposite sex. W1 interventions include facility not to place W1 within arm’s reach of residents of the opposite sex. On or about March 19, 2021, W1 was placed next to Alleged Victim (AV) in common area and W1 inappropriately touch AV. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
