Findings: On or about November 22. 2020, the Alleged Victim (AV) was sent to the hospital for kidney stones and a urinary tract infection (UTI), returned from the hospital with orders to have multiple medical appointments made for him/her, including a visit to a urologist. The facility failed to complete any of the appointments. On March 20, 2021, the Alleged Victim was sent to the hospital and was found to have a UTI with sepsis and kidney stones, placing AV in unreasonable discomfort. The facility failed to ensure physician services for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
