Findings: On or about March 22, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation in which W1 was pushed to the ground and punched by AV resulting in AV sustaining a skin tear to his/her right forearm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
