Findings: On or about March 8, 2021, the Alleged Victim (AV) suffered a fall causing an injury to his/her arm and leg. The facility failed to assist AV up the ramp to the scale when he/she was unable to use his/her walker on the scale. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
